As the only hospital health care organization in Nassau County specifically charged with providing care to the under-served, NuHealth takes its community commitment to very seriously. In fact, bringing essential care, screenings and health information out into the community is one of the most important aspects of our health care philosophy – and a concept that has now become a core element of national health reform. Our top goal is to ensure that every Long Islander can overcome the physical, cultural and financial barriers standing in the way of their good health.

  • NuCare is making it possible for everyone to get the care they need to stay healthy by helping individuals and families find a “medical home” and low or no-cost coverage.
  • Our Family Health Centers in Elmont, Hempstead, Roosevelt, Roosevelt High School, South Ocean Care and Westbury provide medical homes and bring a broad array of services to the communities where they are needed most.
  • Helping Hands offers medical coverage to unemployed individuals and their families.

NuHealth Family Health Centers / LIFQHC

NuHealth’s five Family Health Centers provide essential primary care services and “medical homes” to residents of some of Long Island’s most underserved communities. By bringing quality primary care and a growing number of specialty services out into the community, we’re helping more Long Islanders get the care they need, where they need it most. Our goal is to keep our patients stay healthy and out of the hospital. That helps improve the health of our community and lower the overall cost of delivering health care services…and that’s good for everybody.

Helping Hands

Health Coverage for the Unemployed

The economic crisis in America, while easing slightly, is far from over – especially for the thousands of Long Islanders whose unemployment benefits have recently expired or who have lost full time work and lack health insurance. That’s why NuHealth created the Helping Hands program: to make sure that loss of income does not lead to loss of good health. By enrolling in Helping Hands, our unemployed neighbors can open the doors to a number of health care and coverage options for themselves and their families. We will:

  • Determine eligibility for available government insurance coverage, including Medicaid and New York State’s Family Health Plus and Child Health Plus programs.
  • Provide Medicaid-eligible individuals with a primary care physician if they do not already have one.
  • Enroll those who are ineligible for other government assistance programs in our NuCare program, where they will receive health care on a sliding scale.
  • Set up the applicant and his or her family with a primary care physician for a physical and basic health assessment.
  • Provide financial assistance to help with health care payments.

What is NuCare?
NuCare is new program to help every person in Nassau County get the medical care and support they need to keep themselves and their families healthy. NuCare emphasizes primary care, wellness, healthy living and education, giving patients the tools to take charge of their futures. Instead of worrying about the cost of care and waiting for help in a crowded Emergency Room, you will have a “medical home” with your own doctor, backed by a team of specialists and Care Guides who will provide care for routine illnesses right in your own community.

Who runs NuCare?
NuCare is a program of NuHealth, a public benefit healthcare organization that includes Nassau University Medical Center, A. Holly Patterson Extended Care and a network of Family Health Centers in the community.

Is NuCare health insurance?
No, but we will help qualified participants obtain low or no-cost insurance coverage when available. Even if you do not qualify for insurance, NuCare will make sure you get the medical care you need.

Am I eligible for NuCare?
NuCare is designed to help working families and individuals, many of whom struggle with the high cost of living on Long Island and need assistance to keep themselves and their families healthy.

Take Charge of Your Health
Good health starts with smart, healthy living. NuCare gives you tools to help you make the right choices to keep you and your family’s health on track, including:

  • Healthy Life Handbook – full of information to help you and your family stay happy and healthy (English and Spanish).
  • Health Coaches – bilingual professionals to help you navigate the sometimes confusing health care landscape, advise you how to get and stay healthy, and work with you to set annual health goals, especially for those at risk of chronic disease.
  • Care Managers – specialized professionals to help patients with chronic conditions that require close and careful monitoring, including:
    • Adult Asthma
    • Diabetes
    • Heart Failure
    • Chronic Obstructive Pulmonary Disease (COPD)
    • Hypertension
    • Obesity
  • Healthy Lifestyle and Self-Care Workshops
  • Support Groups for chronic disease management

As a NuCare patient, you’ll have a “medical home” in the closest of our Family Health Centers. That means you’ll be cared for by compassionate doctors and nurses right in your own neighborhood…people who know you and will work to keep you and your family healthy. And, most important, NuCare provides health care you can afford regardless of your income, residency or immigration status. Services you’ll find at our Family Health Centers include:
