2014 Gynecologic Surgery Excellence Award

Nassau University Medical Center achieved recognition as one of Healthgrades 2014 Gynecologic Surgery Excellence Award recipients, the only hospital in Nassau County to receive it 3 years in a row, 2012-2014. The Gynecologic Surgery Excellence Award recognizes hospitals for superior outcomes in surgeries that treat diseases and conditions of the female reproductive system and correct certain conditions of the urinary tract and rectum. Women have fewer complications from their gynecologic surgery and are more likely to survive their stay at hospitals recognized with a Gynecologic Surgery Excellence Award.

2014 Hospital Safety Score

Nassau University Medical Center has received a 2014 Spring Hospital Safety score of "A". The score reflects a hospital's compliance with nationally recognized safe care practices that reduce harm and promote safety for patients. The Hospital Safety Score was created and is administered by The Leapfrog Group, a national leader and advocate in hospital transparency who recommend to consumers going to hospitals that receive an "A" Hospital Safety Score.

2014 NCQA Patient Centered Medical Home

NuHealth’s LI Federally Qualified Health Centers (Elmont, Hempstead, New Cassel, Roosevelt ) and two outpatient practices at the Nassau University Medical Center (Omni Internal Medicine Practice, Omni Pediatric Practice) have received recognition from the Patient-Centered Medical Home 2011 (PCMH 2011) program from the National Committee for Quality Assurance (NCQA) for using evidence-based, patient-centered processes that focus on highly coordinated care and long-term participative relationships.

NuHealth’s Central Line-Associated Blood Stream Infection (CLABSI) Initiative Model was accepted by the Joint Commission and is posted as a leading practice in their TJC Leading Practices Library. It is available to all accredited organizations. Our submission underwent rigorous clinical review and was accepted because it supports quality and safety, and is felt to be of benefit to other organizations. The Leading Practice Library is an important initiative that allows Joint Commission accredited organizations to find “real-life” solutions that have been successfully submitted by their peers.

2013 Healthfirst’s Top Performers for HEDIS - Prenatal and Postpartum Care Measures

NUMC has been identified as one of Healthfirst’s top performers for HEDIS 2013 - Prenatal and Postpartum Care measures. The Healthcare Effectiveness Data and Information Set (HEDIS) is a tool used by more than 90 percent of America's health plans to measure performance on important dimensions of care and service such as the timeliness of prenatal and postpartum care.

2013 AMERICAN HEART ASSOCIATION'S GOLD PLUS QUALITY ACHIEVEMENT AWARD FOR STROKE

NUMC has received the American Heart Association/American Stroke Association's "Get With The Guidelines® Stroke - Gold Plus Quality Achievement Award ". Hospitals receiving Get With The Guidelines Gold Plus Achievement Award have reached an aggressive goal of treating stroke patients with 85% or higher compliance to core standard levels of care as outlined by the American Heart Association/American Stroke Association for 12 consecutive months. In addition, those hospitals have demonstrated 75% compliance to 7 out of 10 stroke quality measures during the 12-month period."

2013 AMERICAN COLLEGE OF RADIOLOGY DESIGNATION BREAST IMAGING CENTER OF EXCELLENCE

Nassau University Medical Center's breast imaging center has been designated by the American College of Radiology's Commission on Quality and Safety and the Commission on Breast Imaging (ACR) as a Breast Imaging Center of Excellence (BICOE). Peer-review evaluations, conducted in each breast imaging modality by board-certified physicians and medical physicists who are experts in the field, have determined that NUMC has achieved high practice standards in image quality, personnel qualifications, facility, equipment, quality control procedures and quality assurance programs.

2013 US NEWS & WORLD REPORT BEST NURSING HOMES IN NEW YORK

For the fourth consecutive year, the A. Holly Patterson Extended Care Facility has been awarded five out of five star rating by the CMS, placing A. Holly Patterson in the top 2% of all nursing homes in the state of New York and among the best facilities in the United States as indicated in a recent "US News and World Report" listing of best hospitals and nursing homes.

2013 AMERICAN COLLEGE OF RADIOLOGY ACCREDITATION

NUMC has earned the American College of Radiology accreditation for the Computed Tomography (Cat Scan) services provided at the medical center. This certification signifies that NUMC's services meet or exceed the national standards in quality and safety in Computed Tomography.

2012 NAPH GAGE SAFETY NET AWARD

The NAPH Gage Awards Program - named after NAPH founder and safety net advocate Larry S. Gage - honors and shares the outstanding work of NAPH members. Winners are recognized for innovations that improve community health and save money and that can be replicated in organizations across the country.

The 2012 Award for Improving Quality and Patient Safety award recognizes member projects that have improved the quality of care delivered to patients and reduced or eliminated harmful patient events.

Winner - NuHealth/Nassau University Medical Center (East Meadow, N.Y.) for Eliminating Central Line-Associated Bloodstream Infections (CLABSIs). To eliminate CLABSIs - one of the most serious hospital-acquired infections - NuHealth improved procedures for inserting and maintaining central lines and monitoring patient risks. The organization used real-time feedback and evidence-based care to reduce CLABSIs by 74 percent in less than a year.

ABSTRACT: Transforming the Process of Care to Reduce Central Line Associated Bloodstream Infections

NuHealth/Nassau University Medical Center (NUMC), East Meadow, NY

Despite a variety of initiatives beginning in 2006 (such as a central line FMEA, utilization of the IHI Central Line Associated Blood Stream Infection [CLABSI] Prevention Bundle, development and implementation of policies and procedures to reflect evidence based medicine, and ongoing surveillance), NUMC could not achieve either a sustained statistically significant reduction or "Zero Level" in our CLABSI rate. Modeling the lessons of high-reliability science, NUMC sought to literally transform the process of central line insertion and maintenance care at the bedside. Beginning with a simple Logic Model we identified behaviors and processes that needed to be changed and/or implemented in order to enhance employee buy-in, produce program deliverables and achieve program success. We then explored the available resources to achieve and maintain our desired outcome. Program challenges were placed into 3 critical pillars (Control, Communication and Accountability) and proposed interventions were identified for each pillar. A dedicated team was formed and charged with implementation and monitoring of the initiative. A program evaluation performed eight months after the project initiation revealed the hospital-wide CLABSI monthly mean decreased 74% from 3.357 to 0.875. NUMC's adult ICUs showed a 97% decrease in the CLABSI monthly mean from 2.46 to 0.07. Aggregated data was analyzed at the end of 20 months and control charts highlighted a statistical shift below the control limit (both in the adult ICUs as well as hospital wide)Additionally, utilizing the Opportunity Estimator (Johns Hopkins Quality and Safety Research Group ), NUMC achieved remarkable outcome performance estimates:

NuHealth has earned The Joint Commission's Gold Seal of Approval for accreditation by demonstrating compliance with The Joint Commission's national standards for healthcare quality and safety in hospitals. The Joint Commission's Gold Seal of Approval™ is recognized nationwide as a symbol of excellence and reflects an organization's commitment to meeting certain performance standards. To earn and maintain The Joint Commission's Gold Seal of Approval, an organization must undergo an on-site survey by a Joint Commission survey team at least every three years.

2012 GWTG STROKE

NUMC has received the American Heart Association/American Stroke Association's Get With The Guidelines®-Stroke Gold Plus Quality Achievement Award. The award recognizes the Nassau University Medical Center's commitment and success in implementing excellent care for stroke patients, according to evidence-based guidelines. To receive the award, NUMC achieved 85 percent or higher adherence to all Get With The Guidelines-Stroke Quality Achievement indicators for two or more consecutive 12-month intervals and achieved 75 percent or higher compliance with six of 10 Get With The Guidelines-Stroke Quality Measures, which are reporting initiatives to measure quality of care.

NUMC has received the Get With The Guidelines®-Heart Failure Silver Quality Achievement Award from the American Heart Association. The recognition signifies that the Nassau University Medical Center has reached an aggressive goal of treating heart failure patients with 85 percent compliance for one year to core standard levels of care as outlined by the American Heart Association/American College of Cardiology secondary prevention guidelines for heart failure patients.

A. Holly Patterson Extended Care Facility was recognized in U.S. News & Worlds annual Best Nursing Home ratings receiving 5 stars, the highest overall rating, from the federal government's Centers for Medicare and Medicaid Services.

2012 DEPARTMENT OF HEALTH AND HUMAN SERVICES (HHS) ORGAN DONATION SILVER MEDAL OF HONOR

NUMC was awarded the HHS Organ Donation Silver Medal of Honor for their success in increasing the number of organs available and transplanted. This recognition signifies NUMC achieved and sustained the national goals for donation, including a donation rate of 75 percent or more of eligible donors at the facility.

2012 IPRO FIVE DIAMOND PATIENT SAFETY STATUS

NUMC's Dialysis program was recognized as a Five Diamond Patient Safety Facility by IPRO's End Stage Renal Disease Patient Safety Initiative. The goal of this program is to help dialysis facilities better implement patient safety principles among both staff and patients.

2012 PRESS GANEY TOP IMPROVER AWARD - PATIENT SATISFACTION

Nassau University Medical Center has won a Press Ganey Top Improver Award for the Emergency Room. This award recognizes clients who have shown continuous improvement - comparing quarterly scores over two years. Winners of the award for patient satisfaction must have shown improved scores in their respective database for at least three of the four eligible periods

2012 DENVER HEALTH SYSTEM LEAN TRAINING PROGRAM

NuHealth was selected by the National Association of Public Hospitals to participate in Denver Health System LEAN Training Program. LEAN program methodology is a comprehensive approach to improving quality and efficiency in organization management and operations.

2012 JOINT COMMISSION BRIEFINGS

NUMC's award-winning central line associated blood stream infections (CLABSI) initiative to improve the blood stream infection rates was included in the February 2012 edition of the Joint Commission publication Briefings. The article identifies the three silos that NUMC used when developing process improvement for CLABSI prevention and the tools and tactics used by organizational in-house surveyors.
